Safe amalgam removal follows a strict protection protocol — internationally known as the SMART protocol (Safe Mercury Amalgam Removal Technique), developed by the IAOMT — designed to minimise that exposure during removal. Why you shouldn’t have amalgam drilled out the ordinary way

Copious water/cooling to reduce the heat that drives vapour release, and sectioning the filling to remove it in chunks rather than grinding it away.

Supplemental air for the patient (nasal) so you are not breathing air from the removal site.

Protective barriers for the patient’s skin and eyes, and full protective equipment for the dental team.

Proper filtration and amalgam separation so mercury is captured rather than released into the air or water.

The goal of every one of these steps is the same: to contain the mercury released during removal, so that far less of it reaches you or anyone else.


What replaces the filling

Once the amalgam is safely removed, the tooth is restored with a biocompatible, metal-free material — for example an advanced composite chosen specifically for its profile, or a ceramic restoration for larger cavities.
